ISO-NCD Vision Statement

Our Mission

We envision communities where everyone has fair and affordable access to quality NCD prevention, care, and mental health services—reducing preventable illness, minimizing complications, and empowering people to live healthier, longer, and more productive lives.

Our Objectives
• To establish Noncommunicable disease (NCD) community care hubs and wellness clinics that provide comprehensive, patient-centered services—integrating NCD prevention and management with mental health, sexual and reproductive health, HIV, TB and Maternal health.

• To equip health workers, Village Health Teams, peers, and NCD survivors with the skills and tools to deliver multi-disease health education, reduce stigma, promote self-care, and expand equitable access to prevention and treatment services.

• To generate and apply locally-driven research, adapt global best practices, and amplify the voices of people living with NCDs to shape evidence-based policies and public awareness.

• To develop and integrate a digital health application that links patients from the community to health facilities, enabling real-time data sharing, patient tracking, and coordinated monitoring across NCDs.

• To develop a community health entrepreneurship model for NCDs to ensures that healthcare becomes both sustainable and community-owned, empowering patients, households, and VHTs as health entrepreneurs.
