Community Screening & Early Detection
Early detection saves lives — and ISO-NCD is taking this message directly to the community. Our screening campaigns identify people at risk of non-communicable diseases such as hypertension, diabetes, cervical cancer, and sickle cell before complications arise. Working with Village Health Teams (VHTs) and local health facilities, our mobile outreach teams conduct regular screening days in schools, markets, churches, and villages. Community members receive free blood pressure and blood sugar checks, body mass index assessments, and counseling on healthy living and early treatment options.
This initiative not only improves awareness but also strengthens the referral link between communities and health centers. In 2025 alone, ISO-NCD screened over 133 people across Nakaseke and surrounding districts — connecting many to care for the first time.

Health Entrepreneurship
ISO-NCD promotes sustainability by empowering community members to become health entrepreneurs. Through our Health Entrepreneurship Program, we train individuals and small groups to identify and run income-generating activities that also improve public health — from selling healthy snacks and hygiene products to offering affordable blood pressure checks within their communities. Participants receive mentorship, basic financial literacy training, and startup support to ensure their initiatives remain viable and impactful. By linking livelihoods with health promotion, ISO-NCD builds both economic resilience and healthier behaviors at the community level.

Health Education & Behavior Change
Lasting change begins with knowledge. ISO-NCD places health education at the heart of our work, empowering individuals and communities to take charge of their well-being. Through radio talk shows, community dialogues, school visits, and workplace wellness sessions, we share practical information on nutrition, exercise, mental health, and the dangers of tobacco and excessive alcohol use. Our approach combines clear, relatable messages with real-life examples that inspire people to act. By engaging Village Health Teams (VHTs) as local educators, we ensure that information is accessible and relevant to everyone — including those in hard-to-reach rural areas.
In 2025, ISO-NCD reached over 1,000 community members through health education activities, with follow-up surveys showing a 45% increase in routine blood pressure checks and healthier dietary choices among participants. These small shifts in daily habits are reducing NCD risks and transforming lives.

ISO-NCD is transforming access to NCD care by establishing Community Care Hubs — local centers where people can receive regular follow-up services without traveling long distances. Each hub is managed by trained Village Health Teams (VHTs) and community nurses who provide blood pressure and glucose monitoring, medication refills, lifestyle counseling, and referrals to higher-level health facilities when needed. These hubs serve as the backbone of our community-based model, ensuring that prevention and management of chronic diseases happen where people live and work. They also provide safe spaces for group education, peer support, and tracking patient progress using simple digital tools.
